Weekes Drive Surgery

With effect from Tuesday 8th May Weekes Drive Surgery will be closing its doors at 6.00pm.  You can still telephone the surgery up until 6.30pm and your call will be answered and dealt with.

Please be aware if you are advised you need to be seen by a GP you may need to visit Farnham Road Surgery.




In line with NHS guidelines the Practice now share some of your medical information so that other health care professionals can access them to provide you with the best medical care.  This is called a Summary Care record (SCR).   The SCR will contain key information about the medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had in the past.


If you have an accident or fall ill, the people caring for you in places like accident and emergency departments and GP out of hours services will be better equipped to treat you if they have this information. Your SCR will be available to authorised healthcare staff whenever and wherever you need treatment in England. Health care professionals will have to ask your permission before they look at your SCR.

Summary Care Record



There are now two ways you can access GP appointments on line – Patient Access and myGP. For information on both of these and how to obtain them please see below or speak to a member of our Reception Team.



With Patient Access, you can now access your local GP services at home, work or on the move. This is a web-based application which has been developed to expand the services available to patients from their GP Practice. It allows you to request services from your Practice online at a time that is convenient to you.

What does it allow me to do?

  • Patient Access is a 24 hour online service that allows you to:
  • Book an appointment with your Usual GP
  • Cancel an appointment with your Usual GP
  • Order repeat prescriptions
  • View sections of your medical record
  • Update your contact details – keep your mobile and other information up to date


How do I receive this service?

To enable access to this service you must first apply. To do this you need to speak to a member of our Reception Team and you will be asked to complete an application and consent form. To complete the application you must supply two forms of identification one of which must be photo ID (such as a passport or driving licence). You will then be given a document which contains your PIN number, Practice ID, Access ID and your NHS Number.

Register On-Line

Once you have received this document you should register your account on-line. (Please keep this document safe until you have registered your account, then you must destroy it). This document will give you a step-by-step guide to registering for this service.

Sign In

Once you have registered for this service you will then be able to sign in. Please note you will need to enter your User ID and Password each time you use the service.

Named accountable GP for all patients

From 1st April 2015 Practices are required to allocate a named accountable GP to all patients.  The named accountable GP is responsible for a patient's overall care at the Practice.  If you wish to know who your named accountable GP is please contact the Practice.  If you have a preference about who your named accountable GP is please mention this the next time you contact the Practice and we will make reasonable efforts to accommodate your request.
